Registrar Request Form

Submit this form to add players and coaches for temporary or permanent placement.

Requests are due the Weds before the weekend game. If you have a Friday request, please also text Cheryl at 321-543-3401.

Leagues have varying rules on Secondary, Guest and Club Pass players.

GCF - Players can be Secondary or Club Pass. Players cannot "play down" to a Div 3 team. Div 1 and Div 2 players may interchange. Divs 1 & 2 cannot play on Div 3 team.

FSPL - Must be Club Pass player.

NPL - an NPL player within the club can be added to the roster as long as there is space. No guest form would be required.

BYSL - player can be either Secondary or Club Pass. Depending on how often the roster changes Club Pass can be easier.

Fundraising

When individual teams seek financial support from outside of the club, this is considered fundraising. Fundraising examples include selling items—such as discount cards, water bottles, or candies. Fundraisers may also include services such as car washes. We encourage individual teams to raise funds for their team needs. When doing so the following policies must be followed.

SCUSC urges parents and team managers to use their best judgment in planning a fundraiser. Consider all pertinent factors such as appropriateness, safety, and amount of time required by the player in its consideration. Most any standard school fundraiser is encouraged.

Funds should be raised for specific team purposes. For example, teams are encouraged to raise funds for tournament fees, travel fees, or the purchase of specific items.

You no longer need to complete a Permission to Travel form for events in the South Region states (Alabama, Arkansas, Louisiana, Georgia, Mississippi, North Carolina, Oklahoma, South Carolina, Tennessee, North Texas, and South Texas). If the tournament requires you to present permission to travel form from FYSA, that approval is now hardcoded at the bottom of the FYSA State Official Roster. When traveling to an out of the state of tournament, make sure you print a roster within three days of travel.

Player/Parent Agreement: will be done through DocuSign, and maintained by Club.

FYSA Medical Release Form: Download and Print from the Team Account, top right corner link. This will prefill all data entered by parents. Then they need to sign.

US Club Medical Release Form: Download and Print from the Team Account, top right corner link. This will prefill all data entered by parents. Then they need to sign.

Injury Forms

SCU Injury Report Form. Player and Coach injuries must be reported to the Club. Complete the form electronically and submit it to admin@spacecoastsoccer.org or give handwritten reports to Directors. The fillable PDF can be typed in and saved.

Possible Concussion Notification. The team/club fills out this form when a players gets a possible concussion either in practice or a game.

Notes: use Deactivate, not Release when removing players. Most tournaments do not allow you to print the official FYSA roster, which they require. After setting up the roster, contact Cheryl for a generated document.

Coach Guidelines

Golf Cart Rules. NO driving on the turf. The Club Golf Cart is for approved use only. The Directors, Erik Miller (facilities) and Vlade Campo oversee its use. Please contact one of them if you find a need to use the golf cart.

Supplemental Insurance

All registered coaches are covered by supplemental medical insurance provided by the organization under which they play. Comp players are covered through FYSA, Comp NPL players are covered through FYSA and U.S. Club, Rec players in BYSL are covered through FYSA, Rec in-house players are covered through USSSA.

If you are injured during a game or at practice, you can file a claim to help cover the costs of medical treatment.
